At PREMIER THERAPY, we are seeking PRN Physical Therapist Assistants to join our partner's skilled nursing facility in Bellefonte, PA. Our team is committed to providing compassionate care and fostering a collaborative environment to support the well-being of our residents.

Responsibilities Overview

• **Collaborative Environment:** Work closely with the therapy and facility teams to ensure comprehensive care for residents.
• **Education and Communication:** Provide education to families and caregivers, as well as conduct facility education and in-service sessions.
• **Care Plan Meetings:** Attend and actively participate in Care Plan Meetings to contribute to the resident's overall care plan.
• **Documentation:** Maintain accurate daily documentation to track progress and ensure seamless communication among team members.
• **Supervision:** Collaborate with supervising Physical Therapists to optimize therapeutic interventions for residents.
