At Cocoon House we partner with young people, families, and communities in Snohomish County to break the cycle of homelessness through outreach, housing, and prevention. As a key member of the Youth Housing team, the Youth Case Manager in Everett works to ensure that youth in the Transitional Living Program are setting and maintaining progress toward goals in various domains including basic needs, life skills, education, employment, health, mental health, and substance use. You will be responsible for developing individualized plans to meet the needs of each youth, as well as providing guidance, advocacy, and professional support. The Youth Case Manager should have a good understanding of the unique challenges facing youth in our community and be able to build strong relationships with them to ensure their success while maintaining healthy boundaries. If you have a passion for helping young people reach their goals, then the Youth Case Manager role could be the perfect opportunity for you!
Cocoon House is committed to continually growing in our equity, diversity, and inclusion journey, and candidates with lived experience or identify as BIPOC and/or LGBTQ+ are strongly encouraged to apply! We will also provide reasonable accommodation to qualified persons with disabilities upon request.
