The Family Medicine Clinic at NMCP are coupled with Deployment Health and devoted exclusively to Active-Duty Care. Our Team takes pride in meeting your health care needs in the Primary Care setting such as preventative, routine, and urgent care.
We employ each Active-Duty Service Member to be engaged in their health care and operational readiness by providing a Primary Care Manager (PCM) to those enrolled into our Family Medicine Clinic.

Women’s Health Specialty Services
Our providers provide women’s health specialty services on a referral basis. Please see your Primary Care Manager for details.
Separations and Retirement Physical Exams
Appointments cannot be booked via Hampton Roads Appointment Center. For patients enrolled in our Medical Home Port, report to Family Medicine Clinic to pick up a physical exam checklist and form packet. Appointments will be booked by clinic staff once paperwork and labs are verified complete.
PARFQ/PRT Screenings
Member must bring PARFQ, PRIMS printout and SF600 to the appointment. These forms are downloaded from PRIMS.
