Binocular stereopsis, or stereo vision, is the ability to derive information about how far away objects are, based solely on the relative positions of the object in the two eyes. It depends on both sensory and motor abilities. In this review, I briefly outline some of the neuronal mechanisms supporting stereo vision, and discuss how these are disrupted in strabismus. I explain, in some detail, current methods of assessing stereo vision and their pros and cons. Finally, I review the evidence supporting the clinical importance of such measurements.

Profound bradycardia during eye surgery is potentially serious event. In clinical practice this Oculocardiac Reflex (OCR) is most often encountered during squint surgery. The objective of this study was to assess the occurrence of OCR and prove the effect of anticholinergic premedication (atropine) to prevent OCR. This study comprises of sixty patients (age 2-30 years) operated for squint surgery under general anaesthesia. Cases were divided into two groups of thirty each. Group 1 was premedicated with intravenous injection of atropine and Group 2 did not receive any premedication. Patients were monitored during operation for any bradycardia or dysrhythmias. The observed data showed occurrence of 70% OCR in Group 2 as compared to only 10% in Group 1. Severe bradycardia in 40% cases of Group 2 needed intervention by IV injection of atropine. ABSTRACT Amblyopia is a developmental ocular disease of childhood-onset which may lead to persistent sequelae into adulthood. Early detection and management of amblyopia usually result in an improved visual outcome. The purpose of this study was to determine the prevalence and pattern of amblyopia in a rural hospital in Ghana. Clinical records of patients seen (from January 2014 to December 2018) at Westphalian Medical Center, Oyoko, Ashanti Region, Ghana, were reviewed retrospectively. Unilateral amblyopia was defined as a two-line interocular difference or more in visual acuity. Bilateral amblyopia was defined as best-corrected visual acuity (BCVA) of Snellen 6/12 or worse in both eyes, with evidence of bilateral ametropia or obstruction of the visual pathway. Following a review of 12,602 patient records, 258 cases of amblyopia were identified. The mean (±SD) presenting age of amblyopic patients was 24.3 ± 16.1 years, with a male-to-female ratio of 1:1.1. The period prevalence of amblyopia was 2.04%. The period prevalence of unilateral and bilateral amblyopia was 1.38% and 0.66%, respectively. The most prevalent form of amblyopia was refractive with a cumulative prevalence of 1.42%. Strabismic and stimulus deprivation amblyopia accounted for 0.36% and 0.21% of all amblyopic cases, respectively. A major cause of amblyopia in this population was refractive error, hence the use of spectacle correction for its initial management. Repeated assessment after an appropriate period of refractive adaptation would elucidate the proportion of amblyopias needing additional treatment modalities. Vision screening for early detection of amblyopia in childhood with accessible and effective management of amblyopia (including refractive correction and occlusion treatment) is necessary to reduce the impact of amblyopia in Ghana.

Objective recording of accommodation dynamics was performed in four patients with divergence excess exotropia (two true and two simulated) and in three visually normal control subjects. The accommodative peak velocity/amplitude relationship was normal in the exotropic patients and in the control subjects. Latency for decreasing accommodation was increased significantly, and overshoot frequency was decreased, in the divergence excess exotropes of the true variety, thus demonstrating slightly slowed accommodation dynamics in this diagnostic group. These subtle deviations in response dynamics reflect central rather than peripheral mechanisms.

Background: This is a retrospective study to determine the outcomes of the surgical correction in long-standing infantile esotropia with cross fixation.
Methods: Medical charts of a group of patients with esotropia who had cross fixation and underwent surgery for strabismus between January 1991 and December 2004 were reviewed. The mean follow-up time was 4.7 years. Binocularity was measured by the Worth 4-dot test and Titmus stereo test. Twenty-six patients underwent surgery for strabismus. Twenty-one patients aged 8 to 26 years with a minimum 3-year postoperative follow-up were included. Five patients were excluded because they were lost to follow-up after surgery.
Results: Bimedial recession and resection of one lateral rectus muscle were performed in all patients. Recession of the inferior oblique muscle with anteriorization was performed in patients who had inferior oblique overaction. Orthotropia was attained in 14 patients, whereas residual esotropia was diagnosed in 5 patients. Two patients were diagnosed as having exotropia. Two patients required a second surgery for dissociated vertical deviation. Overall, 9 of the 21 patients had indications of binocular function and 12 remained the same in their stereoacuity.
Conclusion: Surgical correction of long-standing infantile esotropia with cross fixation in young adults may improve binocular function and allow long-term alignment stability.

The major types of ametropia and visual problems for Hong Kong Chinese and Caucasian children are different. Consequently, the referral criteria developed for Caucasian children may not be applicable to Hong Kong children. Results of our study suggested that the referral criteria for Hong Kong children should be set as hyperopia of ≥+ 2.0 D, myopia of ≥ 1.0 D, anisometropis of ≥ 1.0 D and anisometropia of ≥ 1.25 D. Sensitivity using only these criteria for abnormal refraction in identifying children with amblyopia. Esotropia, exotropia and subnormal vision (< 6/12) was respectively 100%, 84.6%, 45.2% and 95.7%. The overall sensitivity for the identification of visual problems was 86.1 % and the overall specificity was 76%. If a cover test or a Hirschberg test was introduced into the screening battery so that all the strabismic cases were identified, the overall sensitivity would increase to 98.6%.

An apparatus for strabismus detection was designed to allow photographing fundus and corneal reflexes for Brückner and Hirschberg testing, respectively. Photographs of adult subjects with simulated strabismic deviations were viewed by two groups of observers, optometry students and optometry faculty. In general there was no significant difference in testing results between the two groups. Subject error produced unreliable Brückner results. However, Hirschberg testing was effective approximately 80% of the time in detecting a deviating eye in strabismus of magnitudes of about 5 delta. Anisometropia in Brückner testing was also investigated. Magnitudes of 2.00 D or greater significantly affected results. Clinical application of the photographic Hirschberg test may have potential value as a screening method. The photographic Brückner test is not recommended for adults; its reliability in children requires investigation.

Botulinum toxin (BT) prevents the release of acetylcholine at the neuromuscular junction and produces reversible paralysis of striated muscle. This effect was initially used for ophthalmologic indications, particularly strabismus. The idea of using the toxin for purely cosmetic purposes arose from the observation that therapy for facial dystonias markedly decreased lines and wrinkles in the treated areas. The ease of application, high safety profile, and impressive results from the cosmetic use of BT have led to its rapid acceptance by many clinicians. As use of BT increases and its indications expand, an understanding of this therapeutic modality is timely. This article discusses the pharmacologic aspects of BT as well as the methods for administration as they relate to its cosmetic use.

Skew deviation is a vertical misalignment of the eyes caused by damage to prenuclear vestibular input to ocular motor nuclei. The resultant vertical ocular deviation is relatively comitant in nature, and is usually seen in the context of brainstem or cerebellar injury from stroke, multiple sclerosis, or trauma. Skew deviation is usually accompanied by binocular torsion, torticollis, and a tilt in the subjective visual vertical. This constellation of findings has been termed the ocular tilt reaction. In the past two decades, a clinical localizing value for skew deviation has been assigned, and a cogent vestibular mechanism for comitant and incomitant variants of skew deviation has been proposed. Our understanding of skew deviation as a manifestation of central otolithic dysfunction in different planes of three-dimensional space is evolving. The similar spectrum of vertical ocular deviations arising in patients with congenital strabismus may further expand the nosology of skew deviation to include vergence abnormalities caused by the effects of early binocular visual imbalance on the developing visual system.

To investigate the level of agreement between orthoptists and medical practitioners in the comprehensive eye examination of children seen in an orthoptist-led triage clinic. Patient records over a 6-month period were retrospectively reviewed. Those with a presenting complaint related to vision or ocular motility were triaged into the orthoptist-led clinic and included in the study. Patients who did not meet the triage protocol and those who were not assessed by a medical practitioner at a subsequent appointment were excluded from analysis. The clinical findings from the orthoptist and medical practitioner were collected and compared. In total, sixty-three patients were reviewed during the 6-month period and met the inclusion criteria. Orthoptic therapy was instituted in an adult, alternating esotrope with asthenopia who exhibited inaccurate static accommodative responses with adequate, clinically determined, accommodative facility. During and following therapy the patient showed considerable improvement in accommodative accuracy and related visual functions, such as contrast sensitivity, in the absence of any change in strabismic deviation. This case provides the first laboratory documentation of orthoptic effects on accommodation in an adult strabismic.
