Which Multimodal Physiotherapy Treatment Is the Most Effective in People with Shoulder Pain? A Systematic Review and Meta-Analyses

The study aimed to determine if combined physiotherapy treatments offer additional benefits over exercise-only programs for shoulder pain and to identify the most effective combined treatment. A systematic review, registered in PROSPERO (CRD42023417709), and meta-analyses were conducted. Quality analysis was performed using the PEDro scale on randomized clinical trials published from 2018 to 2023. Twenty articles met the inclusion criteria. The most commonly used combination was exercise plus manual therapy, without being statistically superior to exercise alone. The meta-analysis indicated that combining exercise with low-level laser therapy (mean difference of −1.06, 95% CI: −1.51 to −0.60) and high-intensity laser therapy (mean difference of −0.53, 95% CI: −1.12 to 0.06) resulted in the greatest reduction in SPADI scores. Adding manual therapy provided limited additional benefit (mean difference of −0.24, 95% CI: −0.74 to 0.27). Progressive exercise with advice or telerehabilitation yielded modest improvements. The multimodal meta-analysis for DASH scores showed significant improvement (mean difference of −1.06, 95% CI: −1.51 to −0.60). In conclusion, therapeutic exercise is the cornerstone of shoulder pain treatment, with the addition of laser therapy showing substantial benefits. Manual therapy and educational interventions offer some benefits but are not consistently superior. More rigorous studies are needed. Full article

Background: Shorter hip stems have shown promising mid-term results but lack long-term data. High rates of distal cortical hypertrophy (CH) have been described, suggesting a more diaphyseal load transmission. This study aimed to determine patient-specific and surgery-related factors influencing CH and their impact on 10-year outcomes. Methods: It included 100 consecutive total hip arthroplasties (THAs) using the Fitmore stem (Zimmer, Warsaw, Indiana), with clinical and radiographic follow-ups at 1, 2, 5, and at least 10 years post-surgery. Results: No revisions were performed due to aseptic loosening after a mean of 11.6 years (range: 10–13.5 years). CH was observed in 26% of hips, primarily in Gruen zones 3 and 5. There was no significant difference in the Harris Hip Score between patients with and without CH. Larger stem sizes and greater axial subsidence significantly correlated with CH occurrence (OD 1.80, (1.13–1.92), p = 0.004; OD 1.47, (1.04–2.08), p = 0.028). The Fitmore stem demonstrated excellent survival rates and favorable outcomes over 10 years. Conclusions: Despite a lower CH rate compared to other studies, significant correlations with stem size and subsidence were identified. This study underscores the importance of patient selection and achieving high primary stability to maintain the metaphyseal anchoring concept. Full article

Lung resection represents the main curative treatment in lung cancer; however, this surgical process leads to several disorders in tissues and organs. Previous studies have reported cardiovascular, pulmonary, and muscular disturbances that affect the functional capacity of these patients in the short, mid, and long term. However, upper limb impairment has been scarcely explored in the long term, despite the relevance in the independence of the patients. The aim of this study was to characterize the upper limb impairment in survivors of lung cancer one year after pulmonary resection. In this observational trial, patients who underwent lung cancer surgery were compared to control, healthy subjects matched by age and gender. Upper limb musculoskeletal disorders (shoulder range of motion, pain pressure threshold, nerve-related symptoms) and functional capacity (upper limb exercise capacity) were evaluated one-year post-surgery. A total of 76 survivors of lung cancer and 74 healthy subjects were included in the study. Significant differences between groups were found for active shoulder mobility (p < 0.05), widespread hypersensitivity to mechanical pain (p < 0.001), mechanosensitivity of the neural tissue (p < 0.001), and upper limb exercise capacity (p < 0.001). Patients who undergo lung cancer surgery show upper limb musculoskeletal disorders and upper limb functional impairment after a one-year lung resection. This clinical condition could limit the functionality and quality of life of patients with lung cancer. Full article

Background: The clavicle remains one of the most fractured bones in the human body, despite the fact that little is known about the MR imaging of it and the adjacent sternoclavicular joint. This study aims to establish standardized values for the diameters of the clavicle as well as the angles of the sternoclavicular joint using whole-body MRI scans of a large and healthy population and to examine further possible correlations between diameters and angles and influencing factors like BMI, weight, height, sex, and age. Methods: This study reviewed whole-body MRI scans from the Study of Health in Pomerania (SHIP), a German population-based cross-sectional study in Mecklenburg–Western Pomerania. Descriptive statistics, as well as median-based regression models, were used to evaluate the results. Results: We could establish reference values based on a shoulder-healthy population for each clavicle parameter. Substantial differences were found for sex. Small impacts were found for height, weight, and BMI. Less to no impact was found for age. Conclusions: This study provides valuable reference values for clavicle and sternoclavicular joint-related parameters and shows the effects of epidemiological features, laying the groundwork for future studies. Further research is mandatory to determine the clinical implications of these findings. Full article

This study examined injuries sustained by a six-year-old child dummy in a numerical dolly rollover crash using a Toyota Yaris 2010. A harmonic dynamic system (HDS) composed of spring, dampers, and masses with a Low Back Booster (LBB) is denominated as HDS-LBB model. The HDS-LBB was designed to allow damping movements along three Cartesian axes (X, Y, Z) to reduce the energy transferred to the child by a motor vehicle accident and avoid a high injury risk. The HDS-LBB incorporates springs into the vertical axis to decrease the vertical movement during the rollover. The numerical analysis was conducted using LS-Dyna® R12.1 version during an interval of 1 s, and the boundary conditions were set by the Federal Motor Vehicle Safety Standard (FMVSS) 213 for child restraint recommendations and the FMVSS 208 for a dolly rollover procedure. Data on head and thorax decelerations, neck flexion-extension, and thoracic deflection were acquired at a rate of 1 ms. The injury values obtained by the HDS-LBB were compared with the injury values by another configuration denominated LBB-ISOFIX to assess the effectiveness of the model proposed. The results show a higher peak injury value in the neck and thorax because of seatbelt displacement across the child’s shoulder. Nevertheless, despite this seatbelt behavior, the injuries sustained remained below the Injury Assessment Reference Values (IARVS). Full article

Because of systemic discrimination, transgender individuals are at greater risk of being the victims of violence and of homicide. Accurate post-mortem identification from skeletonized remains of transgender individuals must be incorporated into a new standard for forensic anthropological analyses. A critical component of any investigation is the assessment of skeletal remains for evidence of gender-affirming care. A systematic review of the current medical literature was conducted to compile in one document descriptions of changes that could be used by forensic anthropologists to recognize skeletal manifestations resulting from gender-affirming surgeries, including facial feminization surgery (FFS), shoulder width reduction surgery, and limb-lengthening procedures. These skeletal changes, when present bilaterally and without evidence of healed trauma, serve as key indicators of a person’s transgender identity postmortem. Recognizing common patterns in bone structure alterations due to gender-affirming interventions will assist in identifying transgender individuals and providing closure for families. By integrating markers from gender-affirming care practices into forensic investigations, this research contributes to more inclusive and rigorous forensic investigations. Full article

The beach chair position (BCP) is widely used in shoulder surgery; however, it frequently leads to hypotension. Hypotension in BCP is prevalent among older patients who are at risk of secondary complications such as ischemic injuries. Therefore, this prospective study aimed to investigate the association and predictive value of frailty, as assessed by ultrasound-measured quadriceps depth and questionnaire, in patients aged ≥65 years undergoing elective shoulder surgery under general anesthesia. A multivariable logistic regression analysis was performed to identify independent risk factors for hypotension in BCP under general anesthesia. Receiver operating characteristic curves were constructed to assess the predictive values of various parameters. The results indicated that a quadriceps depth < 2.3 cm and BCP for an extended period significantly increased the risk of hypotension. The combined consideration of quadriceps depth < 2.3 cm and frailty demonstrated markedly superior predictive power compared with each factor individually. In conclusion, the study findings facilitate the screening and identification of risk factors for older patients undergoing surgery in BCP, thereby enhancing perioperative management. Full article

Background: Hand burns are involved in 80–90% of severe burn injuries. Even though hands correspond to a small total burn surface area (TBSA) of less than 5%, the loss of their functionality has a significant impact on the patient’s life. Studies that provide long-term results regarding hand functionality after hand burns are scarce. Therefore, this study aimed to investigate functional long-term results in a patient-centric approach regarding burn depth, unilateral and bilateral hand involvement, and (non-)isolated hand burns as potential influencing factors in patients with hand burns. Methods: We conducted a controlled cohort study of patients with burned hands treated at our department between 2005 and 2022. Healthy age-, sex-, and handedness-matched participants were used as controls. Data on the demographics, burn-related injuries, and treatments were collected. For a patient-centric approach, we used the Disabilities of the Arm, Shoulder, and Hand Questionnaire (Quick-DASH) and the Michigan Hand Questionnaire (MHQ) as patient-reported outcome measures for functional long-term evaluation, and the Patient and Observer Scar Assessment Scale (patient scale) for assessing long-term scar quality. Results: We enrolled 61 patients with 88 affected hands and 63 matched control participants. Up to 77.1% of the participants were male, with a mean age of 50.7 (±15.5) years and a follow-up of 8.1 (±4.7) years. The mean TBSA was 13.9% (±15.8), with 72.4% of the hands presenting with deep partial-thickness and full-thickness burns and most of the patients had only one hand affected (55.7%). The hand burn patients perceived significantly worse long-term functional scores in every domain of the MHQ as well as in the “overall function” and “work” of the Quick-DASH. Superficial hand burns negatively affected the two-handed activities of daily living (p = 0.013) and aesthetic appearance (p = 0.005) when both hands were involved. Isolated hand burns were associated with more difficulties in work performance (p = 0.03), whereas patients with bilateral hand involvement perceived more pain (p = 0.025). Conclusions: The patients with hand burns can achieve satisfactory long-term functional outcomes over time, but they do not reach the same long-term hand functionality as the healthy matched control group. Our study revealed that factors such as burn depth, unilateral or bilateral hand involvement, and (non-)isolated hand burns indeed have an impact on certain aspects of perceived long-term hand functionality. Monascus ruber is an important fungus that causes spoilage in table olives, resulting in the darkening of the brine, the softening of the fruit, increased pH, and apparent mycelial growth. This study aimed to evaluate this resistance, providing a model to determine the optimal processing conditions for mitigating fungal contamination and prolonging shelf life without antifungal agents while optimizing pasteurization to reduce energy consumption. The resistance in brine (3.5% NaCl; pH 3.5) from Arauco cultivar green olives imported from Argentina was assessed. Four predictive models (log linear, log linear + shoulder, log linear + tail, log linear + shoulder + tail) estimated kinetic parameters for each survival curve. Log linear + shoulder + tail provided the best fit for 70 °C and 75 °C, with low RMSE (0.171 and 0.112) and high R2 values (0.98 and 0.99), respectively, while the log linear model was used for 80 °C. Decimal reduction times at 70, 75, and 80 °C were 24.8, 5.4, and 1.6 min, respectively, with a z-value of 8.2 °C. The current regulatory processes are insufficient to eliminate M. ruber at requisite levels, considering reduced antifungal agents. Full article

Background: Patients suffering from osteoarthritis particularly complain about pain during day and night as well as loss of function. This consequently leads to impaired quality of life and therefore psychological stress. The surgical therapy of choice is joint replacement. Regarding the outcome after operation, expectations might differ between the patient and the surgeon. This can lead to dissatisfaction on both sides. This study aimed to document patients’ expectations of a planned shoulder joint replacement. The results were compared with assessments made by shoulder surgeons. Methods: In total, 50 patients scheduled for operative shoulder joint replacement were included in this study, as well as 10 shoulder surgeons. Patients were requested to fill out questionnaires preoperatively to provide sociodemographic data, PROMS (Patient-Reported Outcome Measures) with regard to the pathology and their expectations about surgery in terms of pain relief, gain of range of motion, strength as well as the impact on activities of daily and professional life and sports. In addition, surgeons were asked what they thought their patients expect. Results: The most important goal to achieve for patients was to relieve daytime pain, followed by improvement of self-care and the ability to reach above shoulder level. The most important factors for patients to achieve after operation were ‘pain relief’ in first place, ‘movement’ in second and ‘strength’ in third. This also applied to shoulder surgeons, who ranked ‘pain relief’ first, followed by ‘movement’ and ‘strength’. When patients where asked what is most important when it comes to choosing their surgeon, 68% voted for ‘surgical skills’, 28% for ‘age/experience’, followed by ‘empathy’, ‘sympathy’ and ‘appearance’. For surgeons, ‘age/experience’ obtained rank one, ‘surgical skills’ was ranked second, followed by ‘sympathy’, ‘empathy’ and ‘appearance’. Surgeons significantly underrated the factor ‘empathy’ in favor of ‘sympathy’. Conclusions: This study shows that patients’ expectations for shoulder joint replacement and surgeons’ assessments do not differ significantly. Relief from pain and better shoulder movement were crucial for patients to achieve after operation, which was in line with surgeons’ expectations. The most important factor for choosing the surgeon was ‘surgical skills’ for patients, while surgeons thought they would care more about ‘age and experience’. This underlines that patients’ expectations should be taken into account within the preoperative medical interview. This might allow an optimization of compliance of the patients and lead to a better satisfaction on both sides. Full article

This study aims to examine sports injuries among amateur badminton players engaged in recreational activities, establishing a foundation for developing effective injury prevention and training programs. We assessed 504 amateur badminton players (302 males, 202 females) competing in the 2023 National Badminton Tournament in South Korea. This study focused on players involved in doubles matches, omitting singles competitors. Participants were divided into categories based on competitive and training sessions to document the location and type of injuries sustained. During competitions, the most frequently injured body parts were the waist (11.92%), ankle (11.66%), and knees and shoulders (11.53% each). In training sessions, injuries primarily affected the back and knees (12.43%) and shoulders (12.21%). The most common types of injuries during competitions were to muscles (35.60%), ligaments (29.32%), and tendons (12.04%), and during training to muscles (39.91%), ligaments (26.68%), and tendons (13.23%). Injuries resulting from player-to-player contact represented 19.44% of cases, while those involving non-player elements such as facility equipment accounted for 16.47%. There is a critical need to develop targeted injury prevention programs for amateur badminton players to mitigate injuries and enhance performance, thereby helping prevent the progression to chronic injury stages. The Addax nasomaculatus is an African antelope currently considered “critically endangered”. There is little scientific knowledge about the species, which could potentially aid in species conservation. The objective of this study was to determine how hair cortisol concentration and hair color vary according to sex, body site, and season across a whole year in zoo-housed addax. The addax population was housed at Parque Lecocq Zoo, Uruguay. Hair samples were taken from the shoulder and rump in autumn, winter, spring, and summer. Hair cortisol concentration in addax varied according to sex, with males exhibiting a higher hair cortisol concentration than females (1.03 ± 0.05 pg/mg vs. 0.91 ± 0.05 pg/mg, p = 0.016, respectively), with these differences being more significant in the spring (p < 0.0001). Hair cortisol concentration varied across seasons (p < 0.0001), with maximum values observed in summer. The color parameters (L*, a*, and b*) varied across seasons (p < 0.0001). The coat was blacker and yellower in winter and whiter in summer. Males had darker and greener hair color than females (p < 0.05). Sex and season-driven variation in hair cortisol concentration and hair color seem to be important elements to consider in relation to thermal comfort and animal welfare in addax, highlighting the warmer seasons as critical. Background: Movement feedback is used to promote anatomically correct movement patterns. Two primary forms of movement feedback exist: verbal cues and visual cues. There is ongoing debate regarding which type of feedback yields superior effects for learning desired movements. This study investigated how a combination of visual and verbal cues improved shoulder stability in four arm movements, Biceps Curls, Reverse Flys, Rowing, and Shoulder Extensions. Methods: Twelve participants were allocated to three different conditions and instructed to perform four different arm movements: Condition 1 (no specific instructions), Condition 2 (image only), and Condition 3 (verbal cues and image). Measurements of acromioclavicular (AC) joint displacement, and electromyography (EMG) peak and burst duration were taken for each arm movement within each condition. Results: Condition 3 exhibited a significant reduction in AC displacement and prolonged EMG burst duration. Variations in EMG peak and burst duration across different arm movements were attributed to anticipated muscle activation specific to each movement. Conclusions: The combination of visual and verbal cues through the “reConnect Your Dots” movement language was found to improve scapular stabilization and associated muscle activation. This approach to movement patterns practice holds promise for injury rehabilitation and risk mitigation for future occurrences. Full article

The reconstruction of past life based on archaeozoology is a challenging domain that offers a range of valuable details concerning former human and animal populations. Additionally, the ancient era is a source of information for human and veterinary medicine, as well as for other biological sciences. This report highlights a pathological lesion identified during the investigation of a horse skeleton from a pit dated La Tène in Alba County (Romania). The left scapula with lesions was collected from the skeleton of a buried 7–8-year-old male horse. The aforementioned bone underwent gross, radiological, and computerized tomography evaluation. Macroscopically, a collar of supraarticular cancellous hyperostosis was detected, most likely as a consequence of an invasive chronic phlegmonous periarthritis and/or bursitis of the infraspinate muscle following probably a penetrating cutaneous wound in the shoulder region. A suppurative periarthritis\bursitis of the infraspinate muscle situated nearby caused, apparently, supraglenoidian periosteitis responsible for osteophytes and exostoses formation in the neck region of the scapula. The present short-term retrospective study evaluated the implant survival rate and peri-implant bone loss around additive-manufactured titanium implants placed in sinuses grafted with Plenum Osshp (Plenum Bioengenharia, Jundia, SP, Brazil) (70HA:30β-TCP) material. A total of 39 implants were inserted after 23 sinus floor elevation procedures in 16 consecutive patients. Prosthetic rehabilitation included fixed partial prostheses (three units), single crowns (eleven units), and fixed full arches (three units). Clinical and radiographic parameters of implant-supported restorations were evaluated after at least one year of occlusal loading. The implant–crown success criteria included the absence of pain, suppuration, and clinical mobility, an average distance between the implant shoulder and the first visible bone contact (DIB) < 1.0 mm from the initial surgery, and the absence of prosthetic complications at the implant–abutment interface. The overall cumulative implant survival rate was 97.43%. No prosthetic complications at the implant–abutment interface were reported. After one year, the mean DIB was 0.23 mm ± 0.14. Within the limits of this retrospective study, it can be concluded that 70 HA:30 β-TCP allowed stable and reliable bone support to maintain healthy conditions around titanium dental implants produced by additive manufacturing.
