PICCK is excited to share that our article, "Breastfeeding and Hormonal Contraception: A Scoping Review of Clinical Guidelines, Professional Association Recommendations, and the Literature," was recently published in Breastfeeding Medicine. Many contraceptive methods are safe for postpartum use, but patients and providers may be confused as to what impact hormonal contraception has on lactation. We performed this scoping review of the most recent U.S.-based guidelines regarding hormonal contraception on lactation to provide synthesis and recommendations to aid providers in counseling their patients. https://lnkd.in/erEcDEdX

Dr Marise Subrayan shares some insights on long-term contraception and sterilisation: 🏵️ If you’ve completed your family, have been advised that future pregnancies would be unsafe or have decided against having children, then permanent sterilisation or long-term contraception may be a good option for you. 🏵️ Adult women of any age, whether or not they have had prior pregnancies, can opt for permanent sterilisation or long-term contraception options. Studies have shown higher rates of regret if sterilisation is performed before the age of 30. 🏵️ Fully removing your tubes (total salpingectomy) is more beneficial than a tubal ligation (partial salpingectomy), because it has the added benefit of reducing ovarian cancer. This procedure can be performed laparoscopically or via mini-laparotomy. It is 99% effective at preventing pregnancy and allows women to continue to have periods and stop using hormonal contraceptive methods. 🏵️ A Mirena intra-uterine device is more effective at preventing pregnancy than a tubal ligation, and can continue to be used later in life as part of a peri-menopausal or post-menopausal HRT (hormone replacement therapy) treatment plan. The Mirena lasts for 5years and can be inserted in the rooms or in the theatre. It is safe and suitable in patients off all ages, including teenagers, as well as nulliparous patients that have never had a pregnancy. 🏵️ A copper IUD is a non-hormonal option for long-term contraception, lasting 10 years. It is affordable and long-lasting but can cause heavy or painful periods. It can also be inserted in the rooms or in the theatre 🏵️ Hysterectomy, or removal of the uterus, can be performed if the are other gynaecological concerns like fibroids, endometriosis or heavy menstrual bleeding. Hysterectomy can be performed laparoscopically (key-hole surgery) or abdominally (open procedure). 🏵️ Vasectomy has the lowest risk of all permanent sterilisation procedures. It is important to do a 3 month semen analysis post-procedure to confirm that the vasectomy has been successful. Discuss with your Gynaecologist to find the best solution for you and your family. A Bartonella infection can be scary to go through, especially if your child is affected. A case in 2015 saw that a 14-year-old boy had sudden-onset psychotic behavior after infection. Without proper diagnosis, it took 18 months for him to get proper treatment. This disease is transmitted by tick bites, fleas, spiders, and lice. You can also get it from cat scratches and bites. Bartonella symptoms can be bad. They include aggression, fatigue, anxiety, depression, muscle and joint pains, sore soles of feet, and headaches. SIgns include blanching stretch marks that do not follwo dermal lines. To treat these symptoms, Bartonella must be first accurately diagnosed and then treated effectively.
