In the latest Case Records of the Massachusetts General Hospital, an 82-year-old woman was admitted to this hospital because of difficulty walking, falls, and cognitive decline. The patient had been in her usual state of health, living independently with her husband and participating in a walking club, until 8 months before the current presentation, when generalized weakness developed. She began to walk more slowly and was no longer active with her walking club; she felt limited by pain in her feet. Computed tomography (CT) of the head, performed without the administration of intravenous contrast material, revealed moderate ventriculomegaly that was out of proportion to the enlargement of the cerebral sulci (shown in figure). Read more about the case in “An 82-Year-Old Woman with Falls and Cognitive Decline” which includes differential and final diagnoses from guest physicians based on evidence from the test results, by Sharon Chung, M.D., et al., from University of California, San Francisco, and elsewhere: https://nej.md/4aLNcoK

Imagine being told you have a rare health condition for which there is no known cure. Now imagine being told by a doctor that they had absolutely no idea how to help you. This situation is not out of the ordinary for people with fibromuscular dysplasia, or FMD – an uncommon vascular disease found primarily in middle-age women. FMD is signified by the abnormal growth of cells in the walls of arteries, generally characterized by a “string of beads” that can cause the vessels to narrow or bulge. For these patients, UH’s Heather Gornik, MD, Director of the Fibromuscular Dysplasia & Arterial Dissection Program at UH Harrington Heart & Vascular Institute and John B. Lally Family Master Clinician in Fibromuscular Dysplasia and Vascular Care, is an oasis. She leads a dedicated program at UH Harrington Heart & Vascular Institute -- one of the largest such programs for FMD and arterial dissection in the world. Dr. Gornik is also a member of the Medical Advisory Board of the Fibromuscular Dysplasia Society of America and serves on the steering committee of the North American Registry for FMD. The registry now contains about 4,000 patients, and UH follows more than 400 of them. Read more at https://lnkd.in/g7BBMwRP #fibromusculardysplasia #FMD #physicianexcellence #patientsfirst

We are excited to share the results of the latest study from Tension Investigators on endovascular thrombectomy for acute ischaemic stroke patients with large vessel occlusion and large infarct: Using non-contrast CT as the primary imaging tool improved the functional outcome and reduced the mortality of stroke patients. This is a significant finding that could change the management of stroke care. Wide variation in the use of CRT-D across hospitals was associated with clinically significant differences in patient outcomes, according to a recent study using data from ACC's EP Device Implant Registry. "These findings have significant public health and clinical implications, as it suggests that appropriate CRT-D utilization may be a process metric that effectively reflects the quality of care delivered to patients with heart failure undergoing cardiac device implantations." #obesity is a vast chronic health issue we simply cannot afford to ignore. The “traditional” treatments for this disease such as surgery can be morbid, most in fact choose not to undertake it due to the invasiveness, lack of availability and cost. We can reach so more people using endoscopic treatments and pharmacotherapy, of course in addition to surgery when it is indicated. I head the pleasure of speaking with John Stanley on 2GB about some of these opportunities, including the #ESG #endoscopicsleevegastroplasty
Dr Saurabh Gupta, Interventional Gastroenterologist at the San, joined John Stanley on 2GB to discuss non-surgical weight loss procedures available and how to maintain good gut health. Listen here https://lnkd.in/gKtnUr3T
